An in-depth assessment of your identified team(s), clinic, or department that are experiencing the greatest challenges
A written action plan with recommendations tied to each area of opportunity identified in the assessment
An on-going partnership with ITH, including access to our consulting experts, training opportunities, and implementation support to ensure a successful care transformation journey
